Psychotherapy Notes:

If we maintain psychotherapy notes, we will not use or disclose them without your authorization (not applicable to our services).

  • Denial reasons: Information not created by us, not part of our records, or is accurate and complete.

Right to an Accounting of Disclosures

You have the right to receive a list of disclosures we made of your PHI.

  • Timeframe: Covers up to 6 years prior to your request.
  • How to request: Submit a written request.
  • Fees: First accounting in a 12-month period is free.
  • Response time: Within 60 days, extendable by 30 days with notice.

  • Our response: We are not required to agree.
